They are licensed and insured. So are most tattoo parlors. Is it against the law? It may be against their professional code of ethics but I know of no law against it. Some doctors do quite well in a practice that specializes in body alterations. I've even known of a few people who have electively had body parts removed or surgically altered for no reason other than they thought it looked good or it got the body part out of the way. How about if I go into a doctors office and ask that a needle hooked up to a vacuum cleaner be inserted into my midsection and body tissue be sucked out?

Indeed, people also have things inserted into themselves on a purely elective basis because they think it'll make them look better.
You *can* get all kinds of things done on an elective basis. No one is going to arrest the cat dude or the lizard dude for their body modifications, though cat dude got his lips altered so they'd be more like a cat's and lizard dude got his tongue bifurcated. In no way should this be illegal just because you don't find it attractive and are shocked that someone would go through the pain associated with it.
