Families Developing their own Plans Workshop
This one day workshop is designed specifically for
Families of People with Developmental Disabilities and Members of their Support Team
(For example, Independent Service Coordinators, Providers of Residential & Employment or Day Programs, Support Brokers, Direct Care Staff, and Case Managers).
To help understand what it means to use PERSON CENTERED THINKING And PERSON CENTERED PRACTICES When writing individual support plans
Registration in this program will require your attendance all day on:
1. 3/9/10 or 3/10/10, in Nashville, TN. (Clover Bottom Developmental Center).
or
2. 3/11/10, in Knoxville, TN.
Families Developing their own plans WHO SHOULD ATTEND? This program is designed specifically for Family Members and People with Developmental Disabilities who are interested in developing meaningful plans for their services and supports. Social Workers, Services Coordinators, QMRPs, Residential & Day Provider staff are welcome to attend as a member of a team along with the family of a person whom they support.
We encourage families to attend along with two or three additional people who know their family member well and can participate in the first full day of the workshop at a minimum.
There is no registration fee for participants. All costs associated with this program have been provided by The Tennessee Council on Developmental Disabilities.
WORKSHOP CONTENT
This training is designed to help family members and caregivers develop real plans that can be used as the foundation of a person’s Support Plan. Participants will receive a workbook to help them through the planning process step-by-step.
Part 1 – Provides an overview of Person Centered Planning. Learn why you should take the time to develop a Person Centered Plan with your family member. Find out how plans are used. Learn the benefits and differences between traditional planning and Person Centered Planning process. (morning of the first day)
Part 2 – Gathering Information From the Person’s perspective (Builds on the skills taught in Part 1) Learn how to gather important information from your family member and others who love him or her, in order to develop a support plan that is based on things that matter the most in the person’s life.
Part 3 – Gathering information from Others Who Care. (Builds on the skills taught in Parts 1 and 2. Learn how to develop communication charts and gather important information from others in order to develop a plan that is based on balancing what is important to your family member with what is important for your family member.
Part 4 – Writing a Support Plan that is person centered based on information collected. (Builds on the skills taught in three previous sessions) The Trainers will guide you through the process of writing a plan including learning how to organize all of the information gathered, record what you have learned and develop a Person Centered Plan which demonstrates respect and is written in plain language. This “First Plan” can then be used by the Support Coordinator to develop the Individual Support Plan required for some services.

ABOUT OUR FACILITATOR:
Shelley Dumas, Ph.D., a consultant with Support Development Associates, has worked in the field of developmental disabilities for the past 21 years. Dr. Dumas has worked with individuals and families as well as national, state and local agencies and organizations in research, program development and evaluation. She has provided training and technical assistance in the areas of self-determination, person centered practices and family support. More importantly, she is the parent of Katherine Dumas, who lives in the Austin, Texas community and receives services and support from a local service provider. Katherine’s first Essential Lifestyle Plan was developed with Michael Smull ten years ago and has had a significant and positive impact on her quality of life (and her families too!).
Registration must be received by the deadlines listed above. We are limiting participation to first 20 families registering. Pre-registration is required and walk-ins cannot be accommodated.

Certificates of attendance will be provided to those who attend both days of training.
Please Note: While there is no fee for this training, participants are responsible for their own travel, meals, snacks and lodging arrangements. You may want to bring a light-weight jacket or sweater as trainers have no control over room temperature.
